Critical Illness Claims

Second, obtain the following supporting documents:

  • The identity card / birth certificate / passport (including the blank pages in order to get the sequential order – hidden information within the blank pages are not allowed) of the covered person or participant
  • The claimant’s online banking documents such as a bank statement or the claimant’s first page of their passbook (only bank accounts with a 12 digit NRIC number). Note that we’ll make any payments via this account if the claim is approved
  • Any supporting medical reports and/or test results

  • Any police report and/or newspaper cuttings, if the critical illness was caused by an accident

*Note: Photocopy of documents MUST be duly certified by authorised parties, i.e. FWD Takaful Berhad Head Office Executive/Manager or Public Notary or Advocate & Solicitor or Commissioner of Oath or Justice of Peace or Ketua Balai Polis or District Officer or Medical Officer.

In addition, for claims incurred outside Malaysia, the confirmation of claim event and all other related documents issued by the Foreign Authority must be certified by Malaysian Embassy or Public Notary at the incident country. If you have returned to Malaysia, the documents can be certified by relevant country's Embassy in Malaysia.

Medical Allowance Claims

  • The hospital admission bill and receipt, with an itemised breakdown of all charges, the date of admission and date of your discharge from hospital.
  • A valid driving license is required when the Covered person is the driver who involved in the Motor Vehicle accident.

Medical Reimbursement Claims

  • Hospital bill/invoice including the itemised invoices with detailed breakdown of charges, date of admission and date of discharge. The hospital admission bill and receipt, with an itemised breakdown of all charges, the date of admission and date of your discharge from hospital

  • Discharge summary/discharge note with doctor’s name and rubber stamp print, date of admission and date of discharge.
  • All investigation reports (laboratory test, radiology, procedure, etc).
  • Payment advice from other third parties insurer/takaful operator for co-takaful claim.
  • Identity card of certificate owner and covered person.
  • Student confirmation letter from the college/university including the studies tenure with date commenced and date of completion, if the covered person is 19 to 22 years old.
  • Police report and a valid driving license of the covered person when the Covered person is the driver who involved in the motor vehicle accident.
  • Covered person’s passport from page 1 till last page including blank pages (for claims incurred outside Malaysia).

    • The takaful participant(s);
    • The covered person(s);
    • The nominee(s); and
    • The claimant (if s/he is neither the participant, covered person or nominee).
  • Proof of relationship between claimant and covered person (e.g. marriage certificate, birth certificate, child(ren) of claimant, parent of deceased, sibling of deceased and claimant).
  • The claimant’s online banking documents such as a bank statement or the claimant first page of their bank passbook (only bank accounts with a 12 digit NRIC number). Note that we’ll make any payments via this account if the claim is approved.
  • Any burial certificate if available. Please write in if it is not available.
  • A valid Driving license is required when the Covered person is the driver involved in the Motor Vehicle accident.
