Making a claim is a simple three-step process. Check the details of what you need in order to make a claim.

Submission Timeline

  1. Ordinarily we only consider claims that are submitted within 60 days of the event in question. If you wish us to extend this deadline then let us know, in writing, with any supporting evidence or documentation.​

  • 1. Death Claim Form or Claimant’s Statement

  • 2. Medical report duly completed by last attending doctor

  • 3. Original Takaful Schedule

  • 1. Certified true copy of the death certificate

  • 2. Certified true copy of the Burial Certificate (Death only)

  • 3. Certified true copy of the Covered Person / Participant and Claimant’s Identity Card or Birth Certificate or Passport

  • 4. Certified true copy of the proof of relationship between claimant and covered person (eg. Marriage Certificate, Birth Certificate, Children of Claimant, Parent of Deceased, Sibling of Deceased and Claimant)

  • 5. Photocopy of Police Report and/or newspaper cuttings (Accidental death only if available)

  • 6. Post mortem report (Accidental Death only if available)


  • Document must be certified true copy either by an Authorised HSBC Amanah Bank Officer, a FWD Takaful staff*, or a relevant Authorised Officer from Government Office, Bank, Hospital, Police and Legal Professionals.
  • For death in foreign country, claim documents must be translated into the official language authenticated at the appropriated embassy.
  • If there is more than one claimant, all claimants must sign on the claimant statement.
  • If any of the above documents/information are not readily available, please explain in writing and submit it to us.
