Making a claim is a simple three-step process. Check the details of what you need in order to make a claim.

Submission Timeline

  1. We only consider claims that are submitted within 90 days of the event in question.But if you wish us to extend this deadline then let us know, in writing, with any supporting evidence or documentation.​

  • 1. Completed Medical Reimbursement Claim Form Part 1 – Patient Claim’s Form

  • 2. Completed Medical Reimbursement Claim Form Part 2 – Medical Report

  • 3. Hospital Bill/Invoice including the itemised Invoices with detailed breakdown of charges

  • 1. Copy of Discharge Summary/Discharge Note with Dr’s name and Rubber Stamp print

  • 2. Copy of ALL Investigation Report (Laboratory Test, Radiology, Procedure, etc)*

  • 3. Copy of Payment Advice from other third parties insurer/takaful operator for co-insurance claim*

  • 4. Copy of Identity Card of Certificate Owner and Covered Person*

  • 5. Copy of Online Banking Document/Bank Statement/First page of Bank Passbook of Certificate Owner (only bank account registered with 12 Digits NRIC No.)*

  • 6. Copy of Student Confirmation Letter from the College/University including the Studies tenure with date commenced and date of completion (if the child is 19 to 22 years old)*

  • 7. Copy of Valid Driving License (for motor vehicle accident only)

  • 8. Copy of passport from page 1 till last page (For claims incurred outside Malaysia)*


  • Copy of documents MUST be duly certified by authorised parties, i.e. FWD Takaful Berhad Head Office Executive/Manager or Public Notary or Advocate & Solicitor or Commissioner of Oath or Justice of Peace or Ketua Balai Polis or District Officer or Medical Officer or Public Official.
  • In addition, for claims incurred outside Malaysia, the confirmation of claim event and all other related documents issued by the Foreign Authority must be certified by Malaysian Embassy or Public Notary at the incident country. If you have returned to Malaysia, the documents can be certified by relevant country's Embassy in Malaysia.
  • If any of the above documents/information are not readily available, please explain in writing and submit it to us.
  • Both the Date of Admission and Date of Discharge are printed on this Hospital Invoices and Discharge Summary.
