Submission Timeline

  1. Ordinarily we only consider claims that are submitted within 90 days of the event in question. If you wish us to extend this deadline then let us know, in writing, with any supporting evidence or documentation.​

Collect supporting documents

  • 1. Total Permanent Disability Claim Form

  • 2. Original Medical Report duly completed by the attending Physician

  • 3. Original Takaful Schedule

  • 1. Certified true copy of the Covered Person/Participant Identity Card or Birth Certificate or Passport

  • 2. Photocopy of Police Report and/or newspaper cuttings (Accidental cause only if available)

  • 3. Certified true copy of other supporting medical reports and/or test results if available

IMPORTANT NOTE

  • Document must be certified true copy either by an Authorised HSBC Amanah Bank Officer, a FWD Takaful staff*, or a relevant Authorised Officer from Government Office, Bank, Hospital, Police and Legal Professionals.
  • If there is more than one claimant, all claimants must sign on the claimant statement.
  • If any of the above documents/information are not readily available, please explain in writing and submit it to us.

*You can reach our FWD Takaful staff at Level 29 Menara Shell​, 211 Jalan Tun Sambanthan​, Brickfields 50470 Kuala Lumpur​ (Mon – Fri, 9am – 6pm​).
